There are a number of services locally that you are able to make self referrals to.

- Young carers
GP referrals
To request a GP referral:

Tracking a referral
To track an existing referral:
- Contact the Elective Care Hub on 0300 422 6360 who can help with queries relating to referrals, wait times, appointments, and operation dates. The team take calls Monday-Friday 8am to 4pm with extended hours until 6pm on Mondays and Wednesdays and Saturday mornings 8am to -1pm

If your referral was not requested by the surgery please contact the provider directly.
NHS e-Referral
If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.
You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.
